In the scene where Donna goes to the hill to talk to her grandad, she speaks of a man that she has been looking for. Then she speaks about the blue box. Is the mysterious man that has also been sneaking into the Adipose corporation, the same man Donna has been looking for?
We find out later in the episode when Donna and the Doctor finally see each others faces again. They show surprise and shock, and this tells us that they both definitely know each other. Donna had earlier spoke about the man to her grandad that she has been looking for. We also find out that there was another woman that had been with the Doctor and had also been "lost" somewhere with others.
In the scene where the "Nanny" tells the captured reporter the name that she is known by, Foster Mother, she tries to make it known that she has a huge reputation; a reputation that is known for evil. What has the "Nanny" done prior to these actions?
When the "Nanny" finally catches up to the Doctor and Donna, she tells them who she is and what prior job she had. The "Nanny" was inter-galactic nurse for a ship that used humans as surrogates. Her job is highly against many laws in the galaxy and the planet they are currently on. The Nanny has come to this planet and created a company that disguises itself as a business that sells weight loss pills with the unknown literal statement " The Fat Just Walks Away". The Doctor and Donna realize that the fat actually does remove itself from the humans as a small body in which the Nanny collects to raise for "Mom and Dad".
