In the opening scene Rick is in what seems like a deserted area with no people alive. There are plenty of cars but no people. Where is everyone?
There could have been a natural disaster or there could have been a mass evacuation of all the people in his town. Having seen future episodes I know there is a zombie apocalypse of sorts. The little girl that picks up her teddy bear is a giveaway too of what has happened because when she turns around she has part of her cheek is torn off and black slime is dripping out. She runs at Rick and he is forced to shoot him. The zombies may have come through and already killed and eaten everyone.
When Rick wakes up in the hospital and no one is around to help him or talk to him. The hospital appears to be abandoned. Is his city abandoned and how much time has passed?
The zombies have overrun Rick's town and the hospital was locked down when the zombie's tried to attack it. Rick was barricaded in by the hospital bed so the zombies couldn't get to him. The time that has passed is unknown because the clock in his hospital room was no longer working. However, the flowers that Shane brought Rick from everybody were dried up and wilted so it can be assumed that Rick has been alone for roughly 2 weeks if not more. He comes upon a door that appears to be holding back zombies.
