In the opening scene there is a flashback and it appears that the Dollhouse is being locked down because someone named Alpha has escaped or is trying to escape. How did this person manage to escape or even be mentally prepared to escape?
Alpha either was having help from an outside source that allowed him to keep some memories or they programmed him with the abilities to escape when no one was around. Another possibility is that Topher could be going behind the backs of Dollhouse with his programming and keeping little bits of the information in the Actives. Alpha could be trying to infiltrate and destroy the Dollhouse as well.
In the scene where the police officer pulls up and interrogates Boyd Langton and the driver. He shoots the driver and holds Langton hostage. Why did he do this?
The cop may have just been overly suspicious and shot the driver for no reason or the officer had generally great intel that led him to Dollhouse. Another possibility is that the cop works for the man chasing after Echo and he doesn't want Langton to interfere with his "fun" of chasing down and killing Echo.
I like most of the points you make in the answer for your first question, however, I don't think that the one about Topher would happen
