In the opening scene it is complete chaos where the group is located. What has happened and why is everything out of control?
It appears that the Dollhouse either got out of control and the people they were programming rebelled in a way and now people are trying to stop them. Another possibility could be going back to the first episode where Mellie was triggered by the head of Dollhouse over the phone. Maybe that same concept was employed on a much larger scale. This outbreak of mind control could have created a small army of super soldiers that are wreaking havoc on the city its forcing regular people to fight back or go into hiding.
In the scene where the little girl finally reveals that she is not who see seems to be when she tries to kill one of the remaining characters. Who was in the body of the little girl that kept killing everyone?
The girl said that she didn't know how she wound up in that body but is really tired of it and is killing everyone in order to get a decent female body back. She had to be tactically trained in order to use a gun properly and kill people with a flashlight. Also, she knew who the empty body was that showed up also. This points to them knowing each other before everything went downhill.
